  1. Philhealth Office received the completeness of the data on Case Investigation Form (CIF) (Date of Testing, RTP-CR result), Annex E and will accept only those with verified Philhealth
  1. Philhealth Staff encode Claim Summary Form from the given CIF.
  2. After encoding the Claim Summary Form, philhealth staff gives the CIF and Claim Summary Form to Billing Office for the Itemized Billing Statement.

After IBS had done, Claim Summary Form will be sign to Chief of Hospital.

  1. After COH Signature, scan the Claim Summary Form, Itemized Billing Statement, Case Investigation Form, Certification (Annex E) and ready for encoding the complete details from CIF to the System (e-Claim)
  2. Transmit all accomplishment claims to the regional office for review.
